Transformative Grant Bolsters Communications Efforts with Underserved Markets Transitions of Care A Key Area of Focus for DHIN in 2016

Dover, DE — February 25, 2016 — The Delaware Health Information Network (DHIN) and its efforts to improve healthcare through better communication were recognized at the national level last year, with a grant of more than $2.75 million to promote the adoption of health information technology and health information exchange across the continuum of care.

In 2016, a priority under the two-year, performance-based grant from the Office of the National Coordinator (ONC) for Health Information Technology, Department of Health and Human Services (grant number 90IX0001/01-00) is better use of technology in transitions of care to long-term post-acute care and behavioral health facilities.

“The ONC funding will support DHIN’s ongoing efforts to build Delaware’s Community Health Record,” said Jan Lee, MD, DHIN CEO, “and improve the sharing of healthcare data among sectors of the population previously unreached: the long-term post-acute care and behavioral health facilities.”

To advance efforts to improve transitions of care, DHIN has partnered with industry leader VorroHealth. The long-term post-acute care (LTPAC) health information technology vendor will implement a clinical interface – BridgeGateHealth™ – to allow for the transfer of clinical data from LTPAC and behavioral health facilities to DHIN’s Community Health Record.
